Job summary

UnitedHealth Group is seeking a Telephonic Care Navigator to work remotely in Washington or Oregon. The role requires full-time Monday–Friday shifts with 9:00am–5:30pm local time, some overtime, and ~4 weeks of initial training.

You will educate members, coordinate provider appointments, and connect them with resources to improve health outcomes. Responsibilities include outreach via multiple channels, ensuring PCP access, maintaining data privacy, and meeting performance metrics.

Qualifications

• High School Diploma or GED or equivalent work experience.
• Must be 18 years of age or older.
• 1+ years of call center or telephonic customer service experience.
• 1+ years of office setting using the telephone and computer as primary tools.
• 1+ years of healthcare / insurance or social work / community outreach experience.
• 1+ years of experience with analyzing and solving customer problems.
• Proficient with Word, Excel, and Outlook.
• Ability to work from home with high-speed broadband.
• Must attend 4 weeks training period fully.
• Flexibility to work full time 9:00am–5:30pm local time.

Responsibilities

• Performs telephonic outreach to DSNP/CSNP members via multiple channels.
• Educates members on gaps in care and assists with scheduling provider appointments.
• Assists with social determinants of health and links to resources.
• Ensures access to PCP for members.
• Outreaches members in line with program guidelines.
• Meets quality and performance metrics.
• Delivers excellent customer service to members and providers.
• Maintains schedule adherence and good attendance.
• Handles confidential health information per HIPAA and regulations.
