Job summary

WellSense Health Plan is seeking a Clinical Care Manager to provide holistic care management for diverse member populations, including chronic and complex conditions. The role integrates medical, behavioral health, and social determinants of health to improve outcomes, through assessments, care planning, coordination, and education.

Remote, full-time position in the United States with competitive compensation and excellent benefits.

Qualifications

• Bachelor’s degree in Nursing required.
• Master’s or doctoral degree in a behavioral health field required.
• Current state RN licensure required or equivalent.
• Minimum three years of clinical experience with chronic/complex conditions.
• Two or more years in care management, discharge planning, utilization management, or related settings.

Responsibilities

• Perform comprehensive assessments and develop individualized care plans.
• Coordinate services across primary, specialty, behavioral health, and community resources.
• Utilize motivational interviewing to engage members and promote self-management.
• Educate members and caregivers on disease processes, preventive care, and treatment plans.
• Document interventions and track outcomes in care management systems.
