Greater Good Health is seeking a Care Navigator in Alabama, remotely based for residents of AL. You will engage older adults, schedule AWVs, and coordinate with Nurse Practitioners to ensure comprehensive follow-up and continuity of care. The role supports value-based, patient-centered outcomes.

Candidates should have 1–3 years in patient-facing health care, excellent communication, and HIPAA knowledge. Remote work with a usual 8–5 CST schedule; strong teamwork and organization are essential.

Competenze

• 1–3 years in a patient-facing healthcare role.
• Strong verbal and written communication skills.
• Familiarity with PHI and HIPAA compliance.
• Ability to multitask in a fast-paced care coordination environment.
• Experience with scheduling AWVs and outreach.

Mansioni

• Conduct proactive outreach to schedule AWVs, home visits, telehealth, and in-clinic visits.
• Answer inbound calls to schedule AWVs and address questions.
• Coordinate with NPs to manage patient panels and scheduling.
• Retrieve and reconcile medical records from external providers.
• Follow up on outstanding provider orders and ensure results are routed.
