Qualifications

• Bachelor’s degree and 2–4 years of related experience.
• Graduate from an Accredited School of Nursing if holding clinical licensure.
• Current state’s clinical license preferred.

Responsibilities

• Evaluate member needs, barriers to care, resources, and coordinate the care plan.
• Develop or contribute to personalized care/service plans and identify needed providers/resources.
• Provide psychosocial and resource support to members, families, and caregivers.
• Coordinate between member/family and care provider team to ensure timely services.
• May perform on-site visits and educate care managers and members on procedures and benefits.
