Job summary

Qualifications

• Bachelor’s degree and 2–4 years of related experience.
• Graduate from an Accredited School of Nursing if holding clinical licensure.
• Equivalent experience in care management and coordination.

Responsibilities

• Evaluates member needs, barriers to care, resources, and recommends the plan.
• Develops or contributes to personalized care/service plans with providers and resources.
• Provides psychosocial and resource support to members and caregivers.
• Coordinates between the member, family and care provider team for timely services.
• Monitors progress toward care plan goals and collaborates with providers.
• Collects and maintains member information to ensure compliance with regulations.
• May perform on-site visits to assess member needs and collaborate with providers.
• Offers education to care managers and families on procedures and benefits.
• Performs other duties as assigned.
• Complies with all policies and standards.
