Qualificações

• Requires a Bachelor’s degree and 2–4 years of related experience.
• Graduate from an Accredited School of Nursing if holding clinical licensure.

Responsabilidades

• Evaluate member needs and barriers to care to inform plan decisions.
• Develop or contribute to personalized care plans and coordinate with providers and resources.
• Provide psychosocial and resource support to members and caregivers.
• Coordinate between member/family and care team to ensure timely access to services.
• Monitor progress toward care plan goals and update plans as needed.
