Centene Corporation is seeking a care manager to develop, assess, and coordinate care for its 28 million members. You will educate members and families on services, coordinate with providers, and ensure timely access to high-quality care through advocacy and care coordination.

The role includes on-site visits, documentation, and ongoing care plan development. A bachelor’s degree and 2–4 years of related experience are required, with nursing licensure preferred for clinical roles.

Qualifikationen

• Bachelor's degree required with 2–4 years of related experience.
• Graduate from an accredited school of nursing if clinical licensure is held.
• Equivalent experience in care management or related field is acceptable.

• Evaluates member needs, barriers to care, and coordinates the care plan.
• Develops personalized care/service plans and coordinates providers.
• Provides psychosocial support and connects to local resources.
• Coordinates between member/family and care team for timely services.
• Monitors progress and revises care plans with providers when needed.
• Collects and maintains member information in compliance with regulations.
• May perform on-site visits to assess needs and collaborate with providers.
• Educates members and families on procedures and benefits.
• Performs other duties as assigned and complies with policies.
