Responsibilities

• Evaluates the needs of the member, barriers to care, and resources available to recommend the best outcome.
• Develops personalized care plans and identifies providers or resources for care.
• Provides psychosocial and resource support to members and caregivers.
• Coordinates between member/family and care team to ensure services are accessible.
• Monitors progress toward care plan goals and collaborates with providers for revisions.
• Maintains member information to ensure regulatory compliance.
• Performs on-site visits to assess member needs and collaborate with providers.
