Job summary

Centene Corporation is seeking a field-based Care Manager to develop and coordinate personalized care plans for members, with focus on families and community resources in a multi-county service area. The role emphasizes advocate-based care coordination and timely access to services.

The ideal candidate will have a clinical background, bachelor’s degree, and 2–4 years of related experience, with strong communication and organizational skills.

Qualifications

• Requires a Bachelor's degree and 2-4 years of related experience.
• Graduate from an Accredited School of Nursing if holding clinical licensure.
• Clinical licensure preferred for nursing candidates.

Responsibilities

• Evaluates the needs of the member, barriers to care, the resources available, and recommends and facilitates the plan for the best outcome.
• Develops or contributes to the development of a personalized care plan/service ongoing care plans/service plans and identifies providers, specialists, and community resources.
• Provides psychosocial and resource support to members/caregivers and care managers to access local resources or services.
• Coordinates between the member and family/caregivers and the care provider team to ensure services are accessible.
• Monitors progress towards care plan goals and collaborates with healthcare providers for revisions as needed.
• Collects, documents, and maintains all member information to ensure compliance with regulations.
• May perform on-site visits to assess member’s needs and collaborate with providers.
• May educate care managers and members on procedures and care options.
• Other duties as assigned by leadership to meet member and business needs.
• Complies with all policies and standards.
