CenterWell Home Health is seeking a part-time Telephonic Registered Nurse Care Manager to support patients remotely, primarily weekends. You will coordinate post-acute care, educate families, and facilitate admissions and follow-ups with the clinical team.

The role emphasizes HIPAA compliance, strong communication, and empathy by phone, with training and career development opportunities in a compassionate telehealth environment.

Qualifications

• Associates Degree required; BSN preferred.
• Compact state RN license across required states.
• At least 3 years post-acute nursing experience.
• Home health or hospice experience preferred.
• Knowledge of home health, hospice, and palliative care services.
• Excellent verbal and interpersonal skills.
• Communication with empathy over the phone.

Responsabilités

• Be a CenterWell Home Health representative in supporting patients transitioning from service.
• Navigate care options including post-acute offerings, Medicare coverage, and billing issues.
• Make clinical level of care determinations and match needs to service sites or alternatives.
• Follow up and track referral and admission outcomes.
• Assist admissions by acting as an ambassador for eligible patients.
• Provide education regarding Home Health, Hospice, and Palliative Care services.
