Job summary

CenterWell Home Health is seeking a Telephonic RN Care Manager to support patients transitioning into and out of services. You will work remotely, collaborating with the clinical team to facilitate post-acute care planning, education, and care coordination by phone.

You will be responsible for determining appropriate levels of care, guiding patients through Medicare coverage options, and ensuring timely follow-up while maintaining high patient satisfaction.

Qualifications

• Associates degree required; BSN preferred.
• Compact state RN license; additional licensure may be obtained.
• At least 3 years post-acute nursing experience; home health or hospice experience preferred.

Responsibilities

• Support patients post-acute; navigate care options and Medicare coverage.
• Determine clinical level of care and match to service sites; advocate for timely referrals.
• Assist admissions and educate patients about Home Health, Hospice and Palliative Care.
• Contribute to department performance and patient satisfaction goals.
