• Minimum three years of clinical experience with chronic, complex, or high-risk conditions.
• Experience in care management, care coordination, discharge planning or related settings.
• Strong motivational interviewing and member engagement skills.
• Ability to work remotely and collaborate with diverse teams.

Responsabilités

• Complete comprehensive assessments and medication reconciliations to identify needs.
• Develop, implement, monitor, and update individualized care plans.
• Coordinate services across primary, specialty, behavioral health and social supports.
• Educate members and caregivers on disease processes, prevention, and self-management.
• Promote transitions of care and reduce hospital readmissions.
