Job summary

Humana Inc. seeks a Telephonic Care Manager, a Registered Nurse who will help ensure continuity of care for patients transitioning into and out of CenterWell Home Health services.

You will determine appropriate post-acute options and coordinate with clinical teams to support admissions and patient follow-up. The role requires an Associates Degree (BSN preferred), compact RN license, and at least 3 years post-acute nursing experience, with home health or hospice knowledge and strong empathetic

Qualifications

• Associates Degree required; BSN preferred.
• Compact state RN license required; additional state licensures may be obtained.
• At least 3 years post-acute nursing experience; home health or hospice experience preferred.
• Knowledge of home health, hospice, and palliative care services.
• Excellent verbal and interpersonal skills; empathetic communication over the phone.
• Learn and master information related to locations and services of clients.

Responsibilities

• Be a CenterWell Home Health representative in supporting patients who have been discharged or may need post-acute services.
• Navigate care options, post-acute offerings, Medicare coverage, billing issues, and resources.
• Make clinical level of care determinations based on discussions and medical data; match needs to services.
• Assist admissions by acting as an ambassador for patients meeting admissions requirements.
• Focus on placing the right patient to the right care setting at the right time.
• Follow company training on HIPAA, ethics, compliance, and policies.
