Qualifications

• Associates Degree required (BSN preferred).
• Compact state RN license required.
• At least 3 years post-acute nursing experience; home health or hospice preferred.
• Knowledge of home health, hospice and palliative care services.
• Analytical mindset and problem-solving abilities.
• Excellent verbal and interpersonal skills; empathetic communication by phone.

Responsibilities

• Be a CenterWell Home Health representative supporting patients discharged from service or needing post-acute services.
• Provide patient options and education using various tools and methods; handle location lookups, directions and complaints.
• Determine level of care based on discussion, records and clinical data; match needs to service site; assist referrals through admission processes.
• Monitor department performance metrics and ensure patient satisfaction goals are met.
• Assist admissions as an ambassador for patients meeting admission requirements.
• Educate patients about Home Health, Hospice and Palliative Care services and eligibility for alternate services.
