Job summary

CenterWell Home Health is seeking a part-time Telephonic Registered Nurse Care Manager in the United States. You will support patients transitioning into and out of services, determine levels of care, and educate on Home Health, Hospice and Palliative Care.

This role requires strong communication, empathy and problem-solving over the phone. Requirements include an Associate degree (BSN preferred), a compact state RN license, and 3+ years post-acute nursing experience.

Qualifications

• Associates degree required; BSN preferred.
• RN license compact/state licensure required.
• 3+ years post-acute nursing experience; home health/hospice preferred.

Responsibilities

• Assist patients transitioning into/out of services; coordinate post-acute care.
• Determine clinical level of care and refer to appropriate service sites.
• Educate patients on Home Health, Hospice and Palliative Care.
• Support admissions by acting as ambassador for eligible patients.
• Maintain patient satisfaction and participate in mandatory training.
