Alliance Health in North Carolina is seeking a Care Manager II for a full-time remote role covering Cumberland County. The position leads communication among care team members, performs comprehensive assessments, and develops unified plans of care for Tailored Plan recipients, coordinating with providers.

The successful candidate will report onsite for meetings when required and travel weekly within Cumberland County.

Qualifikationen

• Master’s degree in Human Services or related field.
• Licensed in North Carolina (LCSW/LMFT/LCAS/LCMHC/LPA) or RN with MH/SUD/I/DD experience.
• Strong assessment, planning, and communication skills.
• Proficient in Microsoft Office and care-management software.

Aufgaben

• Complete Assessment/Planning: perform enrollment assessments and develop Plans of Care.
• Provide Support and Monitoring: educate members and coordinate services; report on status.
• Complete Documentation: upload documents, update demographics, ensure compliance.
• Travel: travel to meetings with members, providers, and stakeholders as needed.
