Job summary

RHA Health Services, LLC seeks a Care Manager for the Caswell County region. The role involves delivering whole-person, integrated care management through site and home visits, and coordinating with an integrated care team to promote better health outcomes.

The position requires RN or master’s/bachelor’s level qualifications in human services with relevant MH/SU/IDD experience, plus valid driver’s license and ability to travel. Hybrid/remote work with travel supports community-based care.

Qualifications

• Care managers must meet criteria to be a Qualified Professional (QP) per NC licensing.
• RN with required experience or advanced degrees with MH/SU/IDD background is preferred.
• Must possess valid driver’s license and be able to travel within Caswell County area.

Responsibilities

• Deliver integrated care management across sites, homes, and communities.
• Lead tailored care planning and 12-month plan updates.
• Collaborate with Integrated Care Team and external partners to implement care plans.
