Job summary

MediSolution is seeking a Chronic Care Manager for a remote role spanning multiple compact states. You will manage a patient panel, perform chart reviews, outreach, and care coordination to support chronic disease management.

Responsibilities include developing care plans, documenting interactions, and achieving billable encounters targets while attending regular team meetings. Ideal candidates are licensed RNs/LPNs with EMR experience and strong communication skills.

Qualifications

• Graduate from an accredited School of Nursing.
• Current COMPACT license to practice in the state of residence.
• Minimum of two years clinical experience in clinic, Med/Surg, Case Management or home health care.
• Experience with EMR and HIPAA-compliant home care environments.
• Strong communication, organization and time-management skills.

Responsibilities

• Abide by plan of care and orders of the practice.
• Provide prevention and intervention for chronic diseases through coaching.
• Develop positive interactions with patients on behalf of our practices.
• Improve revenue by creating billable Care Management episodes.
• Develop detailed care plans for doctors and patients and attend mandatory meetings.
