Job summary

Community Care of North Carolina is seeking a Care Manager 1 - Non-Clinical to provide statewide care management for Medicaid enrolled members in NC. The role supports integration of services, collaborates with providers, and oversees care planning in a field-based, remote-capable setting.

Ideal candidates hold a health-related Bachelor's degree or RN licensure, with 2 years of child welfare experience preferred.

Qualifications

• . Bachelor's degree in a health-related field or RN licensure.
• . 2 years experience with individuals served by child welfare preferred.
• . Must reside in NC or within 40 miles of NC border.
• . CCM certification preferred.
• . Valid driver's license with current auto liability insurance.

Responsibilities

• Provide integrated care management across physical, behavioral health, and social determinants.
• Complete member assessments considering medical, biopsychosocial, behavioral, and cultural needs.
• Collaborate with members, caregivers, and multidisciplinary teams to develop care plans.
• Educate members and families about diagnoses, medications, resources, prevention, and self-management.
• Monitor progress and adjust care plans to achieve SMART goals.
• Refer to community resources and coordinate services without duplication.
• Document activities in the care management system in compliance with policies.
