Job summary

Community Care of North Carolina is hiring a Care Manager 1 - Non-Clinical for statewide NC coverage. The role supports Medicaid enrolled members with integrated care planning and collaboration with families, providers, and community resources.

Travel within NC is required, and remote work is permitted when not in the field. The position requires a Bachelor's in a health-related field or RN licensure, with 2+ years in child welfare preferred, and NC residency or proximity to the border.

Qualifications

• Requires a Bachelor's Degree in health, psychology, sociology, social work, or related human services field or RN licensure.
• 2 years of experience with child welfare population preferred.
• Must reside in NC or within 40 miles of the NC border.
• CCM certification preferred.
• Maintain a valid driver’s license with current auto liability insurance.

Responsibilities

• Provide integrated care management across physical and behavioral health, LTSS, and related needs.
• Complete member assessments and develop care plans in partnership with members and families.
• Coordinate with primary care providers, guardians, and community resources to ensure seamless care.
• Educate members and families about diagnoses, medications, resources, and self-management.
• Maintain documentation in the care management system and meet productivity expectations.
