Qualificações

• Requires a Bachelor’s Degree in a field related to health, psychology, sociology, social work, nursing.
• 2 years of experience with individuals in the child welfare system preferred.
• CCM certification preferred.

Responsabilidades

• Provide integrated whole-person Care Management.
• Complete member assessments considering total individual health.
• Work with members and caregivers to address barriers to care.
• Monitor quality and effectiveness of interventions.
• Utilize Hospital/Data or Electronic Medical Record systems as available.
