Qualifications

• Bachelor's degree in Nursing required.
• Master’s or doctoral degree in behavioral health required or preferred.
• Associate Degree in Nursing with relevant experience may be considered.

Responsibilities

• Complete comprehensive assessments, medication reconciliations, and condition-specific evaluations.
• Develop, implement, monitor, and update individualized care plans.
• Coordinate services across primary, specialty, behavioral health, and community resources.
• Utilize motivational interviewing to promote engagement.
• Document assessments and interventions in care management systems.
