Job summary

CVS Health Corporation seeks a dedicated Case Management Coordinator for a full-time field-based telework role in Ohio. You will travel within Western Ohio up to 50% to member homes and locations, primarily Monday–Friday 8–5 with flexibility.

You will collaborate with care teams to coordinate services, evaluate needs, and support optimal health outcomes for children and families. The role requires strong experience in behavioral health, child and family services, and proficiency with MS Office

Qualifications

• 2+ years of experience in behavioral health, social services, or human services.
• 2+ years of experience with MS Office Suite applications (Outlook, Word, Excel, SharePoint).
• 2+ years of experience in children\'s mental health, child welfare, or related fields.

Responsibilities

• Collaborate with case management to evaluate member needs and benefits.
• Coordinate and implement care plan activities and monitor progress.
• Escalate quality of care issues and coordinate with medical directors as needed.
• Serve as a single point of contact to assist members and address gaps in care.
