Job summary

CVS Health in Ohio is seeking a Case Management Coordinator for a full-time field-based telework role. The position requires traveling within Central and Southeast Ohio to member homes and other locations up to 50% of the time.

Qualifications

• Bachelor's degree or non-licensed master level clinician required.
• 2+ years of experience in behavioral health, social services, or human services.
• 2+ years of experience with personal computers, keyboard and MS Office Suite (Outlook, Word, Excel, SharePoint).
• 2+ years of experience in children's mental health, child welfare, or related public sector fields.
• 2+ years experience in family systems, case management, or related areas.
• 2+ years experience with Ohio delivery systems and local networks.
• Willing and able to travel within the region up to 50% of the time; some travel to New Albany may be required.
• Reliable transportation; mileage reimbursed per policy.
• Willing to work beyond core hours Monday-Friday as needed.

Responsibilities

• Coordinate and implement care plan activities and monitor progress.
• Collaborate with care managers, Medical Directors and other programs to achieve optimal outcomes.
• Serve as a single point of contact for members and assist with gaps in care and access.
