Job summary

CVS Health Corporation is seeking a Case Management Coordinator for a full-time field-based telework role in Ohio, with travel up to 50% to member homes and other locations. You will support the OhioRISE program, coordinate care plans, collaborate with care teams, and help families access services to ensure quality, compliant care and meaningful outcomes.

The role requires scheduling flexibility, strong communication, and proficiency with care management tools to serve children and families

Qualifications

Bachelor's degree or non-licensed master level clinician required.
2+ years in behavioral health, social services, or human services.
2+ years MS Office Suite experience (Outlook, Word, Excel, SharePoint).
2+ years in children's mental health, child welfare, or related field.
2+ years in family systems, case management, or related areas.
Willingness to travel within Northeast/Northwest Ohio up to 50%.
Hours may extend beyond 8-5, Monday-Friday, as needed.

Responsibilities

Coordinate and implement assigned care plan activities and monitor progress.
Evaluate member needs and align benefits with available programs/services.
Collaborate with care managers, supervisors, and medical directors to address barriers.
Act as single point of contact for members to remediate gaps in care and access.
