Job summary

Qualifications

• Bachelor's degree or higher in a behavioral health, human services, health services, or public health field.
• 2+ years in behavioral health, social services, or human services.
• 2+ years experience with computers, keyboard, and MS Office applications.
• 2+ years experience in children's mental health or related public sector field.
• Willing to travel within Northeast Ohio up to 50% and occasionally to the New Albany office.

Responsibilities

• Provide care coordination and education for members to facilitate healthcare outcomes.
• Coordinate and monitor care plan activities and progress.
• Consult with case managers and healthcare teams to overcome barriers.
• Act as single point of contact for members to address gaps in care.
• Document care management processes in accordance with guidelines.
