Job summary

Assist Point Inc is seeking an experienced RN Care Coordinator to provide telehealth-based care coordination and chronic disease management for patients in the cardiology clinic. You will support PCM, remote care management, patient education, care transitions, and collaboration with the care team to improve outcomes and continuity of care.

The role requires an active RN license, strong communication and documentation skills, and the ability to work independently within an interdisciplinary

Qualifications

• Current, active RN license in the applicable US state.
• Graduation from an accredited nursing program.
• Strong communication, clinical assessment, documentation, and care-coordination skills.
• Ability to work independently while collaborating with an interdisciplinary cardiology team.

Responsibilities

• Conduct telehealth outreach, assessments, and follow-up for patients with chronic cardiovascular conditions.
• Coordinate Principal Care Management (PCM) and remote care-management services for eligible patients.
• Develop, implement, and update individualized care plans with cardiologists and care-team members.
• Review home BP, heart rate, weight, oxygen saturation, and remote-monitoring data.
• Educate patients and caregivers on cardiac diagnoses, medications, and self-care.
