Job summary

Millennium Physician Group is seeking a friendly Care Coordinator to join the Care Management team. The role supports care coordination, transitions of care, chronic disease management, patient outreach, and documentation in an outpatient setting.

Qualifications

• Current Florida LPN license or CMA/RMA certification required or preferred.
• Minimum 3 years of clinical healthcare experience in outpatient settings preferred.
• Experience with chronic disease management and care coordination desirable.
• Proficient in EHR and clinical documentation; strong communication skills.

Responsibilities

• Coordinate care with RN Care Manager and care team to implement care plans.
• Perform monthly chart reviews to identify gaps and opportunities.
• Conduct patient outreach, assess barriers, and support transitions of care.
• Educate patients on chronic disease management and preventive care.
• Document activities, track outcomes, and coordinate community resources.
