Job summary

Millennium Physician Group is seeking a Care Coordinator to support the RN Care Manager in outpatient care, emphasizing care coordination, patient outreach, and education. The role is primarily remote, with occasional in-office work, and targets the Sarasota area patients in the surrounding regions.

Responsibilities include chart reviews, follow-up coordination, and collaboration with physicians, nurses, and social workers to improve outcomes and reduce avoidable utilization.

Qualifications

• Florida LPN license or CMA/RMA certification (preferred).
• LPNs must have an active, unrestricted Florida license.
• Current BLS certification preferred.

Responsibilities

• Assist the RN Care Manager in implementing and monitoring individualized patient care plans.
• Conduct monthly patient chart reviews to identify care gaps and intervention opportunities.
• Monitor and track follow-up appointments, referrals, testing, and care plan goals.
• Coordinate services among primary care providers, specialists, hospitals, and community resources.
• Collaborate with the healthcare team to ensure continuity of care across settings.
• Conduct telephonic patient outreach and assess barriers to care.
• Encourage patient participation in care plans and preventive services.
• Support transitions of care after ED visits, hospitalizations, or SNF stays.
• Assist with post-discharge follow-up, scheduling, and medication reconciliation.
• Provide patient education on chronic disease management and self-management strategies.
• Maintain accurate documentation in the EHR and manage medical records.
