Job summary

Acentra Health is seeking a Care Coordinator to support field care coordination outreach and care management program delivery for plan enrollees who aim to promote healthy lifestyles and reduce chronic illness impacts.

The role requires 4+ years in care coordination (field preferred), with telephonic outreach and Spanish language skills being a strong plus. This remote position within Oregon may require up to 5–10% travel to the Tualatin office as needed.

Qualifications

• Associate’s degree in health-related field and/or medical assistant training.
• 4+ years of experience in care coordination (field environment preferred).
• 2+ years of experience in clinical environment, preferably in field delivery.
• Telephonic outreach or call center experience is a plus.
• Bilingual in Spanish strongly preferred.

Responsibilities

• Handle inbound and outbound calls and ensure delivery of comprehensive services to enrollees.
• Participate in interdisciplinary case reviews for coordinated care planning.
• Support quality management activities and assist in meeting program accreditations.
• Assist in budget adherence and report work hours and expenses accurately.
• Adhere to corporate policies including HIPAA, Privacy and Security Rules.
