• Reliable transportation and the ability to travel up to 25% within assigned territory to meet with members and providers.
• Reside within the state of Colorado.

Responsibilities

• Serve as a consultant to care coordination teams.
• Maintain confidentiality and comply with the Employee Handbook.
• Participate in Interdisciplinary care team meetings and community outreach.
• Coordinate referrals to improve health, social, and environmental conditions.
• Develop plans and maintain a reporting system for outcomes.
