Job summary

TimeDoc Health, Inc. is hiring Care Coordinators to support patients with chronic conditions through CCM. This is a phone-based, productivity-driven role with most workday spent on the phone, coordinating care and documenting interactions.

The position is full-time and remote, with a fixed schedule and daily productivity metrics. You will work in a remote environment managing care plans, HIPAA compliance, and collaboration with providers.

Qualifications

• Minimum 1–3 years of CMA/RMA or LPN experience.
• Active certification or multistate license for LPNs.
• Strong verbal and written communication in English.
• Comfortable being on the phone most of the day.

Responsibilities

• Conduct monthly CCM phone calls with assigned practice.
• Make 30–50 outbound calls per day.
• Complete patient interactions to meet CCM goals.
• Educate patients on chronic conditions and care plans.
• Coordinate care with provider offices, pharmacies, and community resources.
• Identify gaps in care and social determinants of health.
• Create and update personalized care plans in the EMR.
• Accurately document all patient interactions.
• Maintain HIPAA compliance and patient confidentiality.
