Job summary

Cosán Group is seeking a compassionate Care Coordinator to work remotely (US). You will manage patient panels, provide health coaching through outreach, and collaborate with clinical teams while ensuring HIPAA-compliant communications.

The role emphasizes building rapport, navigating multiple tech platforms, and advocating for patient needs to improve health outcomes. A HIPAA-compliant home office and 50 Mbps down / 5 Mbps up are required.

Qualifications

• Must hold a nationally recognized MA certification (CMA, RMA, CCMA, NCMA, CMAC, or equivalent).
• OR Certified Pharmacy Technician (CPhT).
• Minimum 1 year of clinical experience in primary care, hospital, or specialty healthcare setting.
• Experience with EMRs and strong computer navigation skills.
• HIPAA-compliant home office setup with high-speed internet.
• Must be legally authorized to work in the United States.

Responsibilities

• Manage a panel of patients and conduct monthly outreach for continuity of care.
• Make 50–60 daily outbound calls to provide care plan support and health coaching.
• Collaborate with clinical teams and caregivers to address clinical and social needs.
• Document all patient interactions in real time using the care coordination platform.
