Job summary

Cosán Group is seeking a compassionate Care Coordinator to manage a panel of patients and provide health coaching through monthly outreach. This 100% remote role requires a private HIPAA-compliant home workspace and legal authorization to work in the United States.

The position focuses on closing gaps in care and coordinating with clinical teams while ensuring high-quality patient communication. You will conduct monthly outreach, perform daily calls, and document interactions in the EMR,

Qualifications

• Must hold CMA, RMA, CCMA, NCMA, CMAC or equivalent MA-C certification.
• Minimum 1 year of clinical experience in primary care, hospital, or specialty healthcare setting.
• HIPAA-compliant home office setup with minimum 50 Mbps download / 5 Mbps upload.
• Experience with Electronic Medical Records (EMRs) and navigation of multiple technology platforms.

Responsibilities

• Manage an assigned panel of patients with monthly outreach and health coaching.
• Collaborate with clinical teams and caregivers to identify and address clinical and social needs.
• Document patient interactions in real time in the EMR and maintain HIPAA compliance.
