Job summary

Cosán Group is seeking a compassionate Care Coordinator to manage a large patient panel and provide health coaching from a HIPAA-compliant home office. This 100% WFH role offers monthly outreach targets, daily patient outreach, and collaboration with clinical teams to close gaps in care.

You will work in a fast-paced environment with exposure to EMR systems across multiple platforms, ensuring accurate documentation and patient advocacy. Local eligibility to work in the United States is required.

Qualifications

• Requires CMA/RMA/CCMA/NCMA/CMAC or MA-C certification or CPhT certification.
• Minimum 1 year clinical experience in primary care, hospital, or specialty care.
• Proficient with EMRs and able to navigate multiple technology platforms.

Responsibilities

• Outreach to manage a panel of 400–450 patients with monthly touchpoints.
• Make 50–60 daily outbound calls for care plan support and coaching.
• Document all interactions in real time while maintaining HIPAA compliance.
• Collaborate with clinical teams and caregivers to address clinical and social needs.
• Support Chronic Care Management, Behavioral Health Integration, and remote monitoring services.
