Cosán Group is hiring a compassionate Care Coordinator to manage a large patient panel and provide health coaching through monthly outreach. This 100% remote role is performed from a HIPAA-compliant home office in the United States.

You will conduct daily outreach, support care plans, and collaborate with clinicians to close gaps in care while ensuring privacy and compliance. Strong communication and EMR skills are essential.

Qualifications

• Must hold CMA, RMA, CCMA, NCMA, CMAC, or equivalent MA-C.
• OR Certified Pharmacy Technician (CPhT).
• Minimum 1 year clinical experience in healthcare.
• Experience with EMRs and strong computer navigation.
• HIPAA-compliant home office setup with fast internet.
• Must be legally authorized to work in the United States.

Responsibilities

• Manage an assigned panel of 400-450 patients and monthly outreach.
• Conduct 50-60 daily outbound calls to support care plans.
• Maintain call quality and professional communication.
• Collaborate with clinical teams to address needs and gaps in care.
• Document interactions in real time and ensure HIPAA compliance.
