Job summary

Cosán Group is seeking a compassionate Care Coordinator to manage a large patient panel and provide health coaching via monthly outreach. You will collaborate with clinical teams to close care gaps and advocate for patient needs.

This 100% remote role requires a HIPAA-compliant home office, strong EMR skills, and the ability to navigate multiple platforms while ensuring compliant documentation and timely outreach.

Qualifications

• Nationally recognized medical assistant certification or equivalent MA-C credential.
• Minimum 1 year clinical experience in primary care, hospital, or specialty setting.
• Experience with EMRs and strong computer navigation skills.
• HIPAA-compliant home office setup with 50 Mbps down / 5 Mbps up.
• Must be legally authorized to work in the United States.

Responsibilities

• Manage an assigned panel of 400-450 patients and conduct monthly outreach.
• Conduct 50-60 daily outbound calls to patients with care plan support and health coaching.
• Collaborate with clinical teams and caregivers to address clinical and social needs.
• Document all patient interactions in real time and maintain HIPAA compliance.
