Job summary

CentraCare Health is seeking a Care Coordinator (RN or LSW) to support payer contract requirements and quality outcomes. The role includes remote work with occasional home visits and on-site team meetings to coordinate services for Medica MSHO.

Responsibilities include coordinating services, partnering with patients and providers, serving as liaison with payers, assessing eligibility, and advocating for patient-centered care to meet payer requirements.

Qualifications

• Bachelor’s degree in nursing or social work.
• Current Minnesota RN or LSW licensure.
• MnChoice Assessor within 30 days of employment.
• AHA BLS Healthcare Provider card.
• Minimum of two years’ experience as RN or SW.
• Valid Minnesota driver’s license with good driving record.

Responsibilities

• Coordinate health and long-term care services covered by payer programs (Medicare/Medicaid/waivers).
• Partner with patients, families, providers, payers and community resources to ensure services.
• Serve as liaison between payer and health system for seamless care.
• Assess eligibility and identify services for long-term care needs.
