• Address member and provider inquiries related to care plans, ensuring continuity of service.
• Document and maintain member records in compliance with state and regulatory requirements.
• Identify member needs and make referrals to Care Managers and community-based organizations.
• Perform other duties as assigned, adhering to all policies and standards.
