Centene Corp. is seeking a Care Coordinator III to support the care management team and engage members with care plans and resources. This remote-position candidate must live in/around Sacramento or Los Angeles, CA.

The role involves outreach by phone or home visits, coordinating with providers and caregivers, handling inquiries, and ensuring continuity of care while meeting regulatory standards. Education and express willingness to educate members on benefits are essential.

Formación

• Requires a High School diploma or GED.
• 2–4 years of related experience in care coordination or case management.

Responsabilidades

• Provide outreach to members via phone or home visits to discuss care plans and next steps.
• Coordinate care activities with providers, partners, and family members to ensure continuity of care.
• Respond to member and provider inquiries related to care plans and services.
• Document and maintain member records per state/regulatory requirements.
• Triage, adjust, and escalate complex requests to management as needed.
• Educate members on benefits, resources, and care options.
• Assist with training or development needs as required.
