• Document and maintain member records in compliance with state and regulatory requirements.
• Identify member needs and make referrals to Care Managers and community-based organizations.
• Perform other duties as assigned, adhering to all policies and standards.

Immediate need for a talented Care Coordinator II. This is a 06+ Months Contract opportunity with long-term potential and is located in US (Remote).

Pay Range: $23/hour. Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).

Key Responsibilities:
