Job summary

Pyramid Consulting, Inc. is seeking a Clinical - Care Navigator for a 06+ months contract, mostly remote within the U.S. You will manage a caseload, assess needs, and connect members with community resources to support access to care.

Required experience includes 2–4 years in care coordination or related field and a relevant degree or clinical certification. Travel for local visits may be needed.

Qualifications

• Degree or background in social work, healthcare, human services, psychology, or related field.
• Relevant professional or clinical certification required.
• 2–4 years of experience in care coordination, case management, outreach, or social services.

Responsibilities

• Manage an assigned caseload and conduct outreach to understand needs and barriers to care.
• Assess social, healthcare, and resource needs; connect with community-based programs.
• Support development and implementation of individualized care plans with care managers.
• Coordinate referrals and follow up to ensure access to resources.
• Conduct in-person member visits and local field visits as required.
• Build relationships with community organizations and healthcare providers.
• Attend community events to identify resources and expand networks.
• Maintain documentation of interactions, assessments, referrals, and follow-ups.
