Job summary

TailorCare is seeking a Care Coordinator for our remote-first team. You will be the first point of contact for patients, handling inbound calls, transferring issues, and facilitating communication between internal and external care teams to deliver compassionate, patient-centered care.

This non-exempt, hourly role emphasizes listening, problem-solving, and coordinating with care teams. You will document interactions, manage referrals, and support administrative tasks to maintain smooth care

Qualifications

• High School Diploma / GED (or higher) is required.
• Experience with escalated customer service issue resolution.
• Knowledge of Google Suite products (Docs, Gmail, Calendar, Slides, Sheets) and basic analytics.
• Phone/Fax system experience.
• EMR experience, Athena & Epic preferred.

Responsibilities

• Answer patient calls and transfer escalated issues to the appropriate team member or resources.
• Identify, request, and collect necessary documentation from patients and providers; support care teams with non-clinical tasks.
• Accurately document patient interactions and ensure timely follow-up and task completion.
