Job summary

Vaya Health is seeking a Care Manager Extender to support Tailored Care Management across North Carolina. The role enhances workforce capacity, supports care managers, and ensures member contact requirements are met while integrating with the care team.

The Extender will receive Tailored Care Management training and must reside in NC or within 40 miles of the NC border, with remote work possible for NC counties served. This is a non-exempt role with overtime eligibility.

Qualifications

• High school diploma or GED is required; must be 18+ and Tailored Care Management training.
• One of: lived experience with an I/DD or a TBI; or Certified Peer Support Specialist; or a parent/guardian of such an individual.
• Has two years of paid experience performing the types of functions described in the Duties/Responsibilities section plus Tailored Care Management experience.

Responsibilities

• Support care managers by performing outreach and follow-up with members.
• Coordinate services/appointments (reminders, transportation).
• Educate members on chronic conditions and self-management strategies.
• Document interactions and activities in the identified health record per standards.
