Qualifications

• High school diploma or GED required.
• Must be at least 18 years old.
• One of: lived experience with I/DD/TBI; Certified Peer Support Specialist; parent/guardian with relevant experience; two years of related paid experience plus one year with Tailored Care Management population.

Responsibilities

• Assist care managers with Tailored Care Management activities.
• Coordinate services/appointments and transportation reminders.
• Provide education on chronic conditions and self-management strategies.
• Share information with the care team and document interactions.
• Assist with referrals and access to community resources.
• Participate in case conferences and help address unmet health-related needs.
