Job summary

Oscar is hiring a Care Coordinator to serve as the central entry point for the Care Management department. You will coordinate care for members across CCM, RAP, and other programs, handling post-discharge transport and inbound inquiries to support one-call resolution.

The role requires 1+ years in healthcare care coordination, strong computer literacy with EMRs (Medecision) and telephony (InContact), and excellent interpersonal communication. Remote role with travel may be required.

Qualifications

• 1+ years of experience with care coordination and navigation in a healthcare setting.
• Proficient computer literacy with EMRs and telephony systems for seamless workflows.
• Excellent interpersonal communication and listening skills.

Responsibilities

• Be the primary bridge and entry point for all members and providers with Care Management.
• Field inbound calls from members and providers with optimized routing and one-call resolution.
• Coordinate care across CCM, TOC, and unassigned populations across multiple settings while maintaining confidentiality.
• Manage non-emergent medical transportation via UberHealth following program guardrails.
• Handle internal escalations to Claims, Eligibility, and Care Navigation.
• Lead special projects as subject matter experts.
• Collaborate cross-departmentally to create a positive member experience.
• Perform additional tasks as needed.
