Job summary

Essen Health Care is seeking an Integrated Care Coordinator to link patients with medical services across our multispecialty network. You will coordinate primary, specialty and behavioral health, and work to remove barriers for underserved populations in the Bronx, delivering patient-centered care.

The role emphasizes care planning, outreach, and collaboration with a team to ensure smooth transitions across telehealth, in-person visits, and home care, while supporting patients with navigation

Qualifications

• High School Diploma or equivalent.
• 1-2 years of direct patient care experience.
• Experience in Care Coordination or Case Management.
• Experience with Remote Patient Monitoring.
• Familiarity with Electronic Health Records (EHR).
• Previous outreach or social services experience.

Responsibilities

• Develop and maintain care plans addressing medical, behavioral, and social determinants.
• Coordinate services across urgent, primary, and specialty care.
• Facilitate transitions between telehealth, in-person visits, and home care.
• Track patient progress with Remote Patient Monitoring.
• Educate patients and families about conditions and self-management.
• Advocate for patients to access services.
