Job summary

Oscar is seeking a Care Coordinator to join our team. This remote role supports the Care Management department, guiding members through CCM, RAP, and other programs while ensuring efficient, compassionate service.

You will manage post-discharge transportation needs, handle inquiries with a one-call resolution mindset, and collaborate with RN Care Managers to improve health outcomes and control costs. Candidates in FL are welcome.

Qualifications

• 1+ years of care coordination experience in healthcare.
• Strong computer literacy; proficient with EMRs (e.g., Medecision) and telephony systems (e.g., InContact).
• Excellent interpersonal and listening skills.

Responsibilities

• Be the primary bridge and entry point for members and providers.
• Field inbound calls with optimized IVR for one-call resolution.
• Coordinate care across CCM, TOC, and other populations.
• Arrange non-emergent transportation via UberHealth following guardrails.
• Escalate to Claims, Eligibility, Care Navigation as needed.
• Lead special projects and collaborate cross-departmentally.
• Perform other duties as assigned.
