Care Coordinators serve as the central entry point and operational backbone for the Care Management department. They provide comprehensive care coordination for members, including those in Complex Care Management (CCM), Readmission Prevention (RAP), and those who do not fit into existing highly specialized programming. By taking ownership of administrative tasks, managing post-discharge transportation needs, and resolving inbound inquiries with a focus on one-call resolution, the Care Coordination Team allows our RN Care Managers to operate at the top of their license and helps members reach their optimum health goals in the most cost effective way.
This is a remote position, open to candidates who reside in: Arizona; Florida; Georgia and Texas. While your daily work will be completed from your home office, occasional travel may be required for team meetings and company events.
The base pay for this role is: $17.91 - $23.51 per year. You are also eligible for employee benefits, monthly vacation accrual at a rate of 15 days per year.
