Job summary

All Metro Health Care - NY is seeking an On-Call Care Coordinator Associate to ensure patient care needs are met efficiently. You will serve as the main liaison for patients, families, and healthcare professionals, guiding access to services and coordinating appointments.

You will monitor schedules, update care plans, and communicate critical information while maintaining records. A bilingual candidate with 1+ year of customer service and home healthcare experience is preferred, with experience

Qualifications

• High school diploma or equivalent required.
• 1+ years of customer service experience required.
• 1+ years of home healthcare experience preferred.
• Experience using HHAeXchange strongly preferred.

Responsibilities

• Serve as the primary point of contact for patients, families, and healthcare professionals regarding care coordination.
• Address inquiries, concerns, and provide guidance on accessing services and resources.
• Schedule patient appointments, coordinating availability of healthcare professionals and patient preferences.
• Monitor and update schedules to accommodate changes, cancellations, and rescheduling needs.
• Communicate pertinent patient information, including diagnosis, treatment plans, and care instructions, to the appropriate individuals.
• Maintain accurate and up-to-date electronic and physical records of patient information, appointments, and communication.
• Monitor HHAeXchange dashboard in-between client/caregiver calls.
• Provide compassionate and empathetic support to patients and their families throughout the care coordination process.
